DIRECTIONS

We are located at 151 York Blvd. at the corner of Bay Street North, and directly across the street from Copps Coliseum. We are on the second floor, Suite 203, and can be reached by stairs or elevator.
Parking is free and is available to all our patients anywhere in our parking lot. Simply bring your parking voucher, if issued via the machine, to the office for validation.
You can enter the parking lot via Bay Street South, York Blvd., or Napier Avenue. If you have any problems in finding a spot if the lot is full, simply ask the attendant for assistance and mention you are going to York & Bay Dental Office for an appointment.
FEES AND PAYMENT
Our office uses the current Ontario Dental Association fee guide to determine cost of dental treatment, and we make every effort to keep any increases to a minimum.
All patients are responsible for the entire cost of all visits to our office. We accept: VISA, Mastercard, Direct Debit(Interac), cheques, and cash. With major dentistry, financial arrangements can be made so patients can receive the dental care they need, and ultimately preserve their oral health without compromise.
Because of the rising costs of billing, a service charge is placed automatically on balances over 30 days.
DENTAL BENEFIT (INSURANCE) PLANS
Many patients have dental benefit plans to help cover their costs. These plans are intended to cover some but not all of the cost of dental care. Most plans include co-insurance, deductibles, or other expenses. These are the responsibility of the patient.
The Ontario Dental Association, the Royal College of Dental Surgeons, plus the Canadian Dental Association all recommend that dentists not accept direct payment from insurance companies.
All patients will receive an itemized receipt and universal insurance claim form for dentistry received in our office at each visit. We fill out all insurance claims and estimates, so make certain your benefit plan information is current (Group number, Plan number, etc) to assure that you receive the benefits to which you are entitled. Our forms contain all the information needed for dental benefit companies to process your claim once we have received this information from you.
